Chronic Kidney Disease (CKD) Program

Our Chronic Kidney Disease (CKD) program gives you one-on-one help. We work with you and your kidney provider to help you stay as healthy as you can so you can enjoy your life. Having CKD means you will need a lot of special care. You will have a team of providers working with you. They will help find the best care plan for you.

What we offer

As part of the program you will get:

  • A care plan just for you
  • Group education classes or one-on-one education sessions
  • Nutritional advice on foods, fluid, and salt that will help you manage the disease
  • Educational materials to take home
  • Access to other CareMore programs, such as our Wound Care Program

The chronic kidney disease program will teach you how to:

  • Take care of yourself
  • Live with your disease
  • Manage your medicines
  • Eat healthy and what foods can control symptoms
  • Deal with more serious problems that may come up

More about Chronic Kidney Disease

CKD is a long-term condition. This means that your kidneys have damage that will not go away. The damage stops your kidneys from cleaning your blood.

Some symptoms of chronic kidney disease are:

  • Muscle cramping at night
  • Feeling tired and having less energy
  • Not wanting to eat
  • Swollen feet and ankles
  • Having puffy eyes mainly in the morning
  • Urinating a lot, mainly at night
  • Having a hard time paying attention
